cat 3550 enable secret mystery

Has anyone had any issues with enable secrets mysteriously appearing on the switch? On a few occasions, the switch asks for an enable secret when none was configured. Each time I have to perform a password recovery. Not sure what's going on. At least I'm becoming an expert at 3550 password recovery. I don't think I'm configuring a secret and then forgetting. I am running the following IOS on the cat:
 
c3550-i5q3l2-mz.121-9.EA1c.bin
