On R4 and R5, why do you have two Multilink interfaces and also two seperate Virtual-Templates configured?
From your config on R4 & R5, the only DLCI's that are bound to your Virtual-Templates are ones that point back to R3.  But the config only has one DLCI/VI bound to each Multilink Group.  You should have both layer 2 paths to R3 configured to belong to the same Virtual-Template and Multilink Interface.  Right now, the Multilink interfaces are actually "Single"link interfaces.;-)
Also, I'm assuming that part of your R3 config was not pasted to the email.  I do not see the DLCI's from the second serial connection to the frame-relay network.
