Sadiq,

If your phone is connected to a Cisco CCM then look at the phone
configuration page. There are some parameters. I had this issue with a few
phones. In my case this was happening coz phone switch was receiving a
Gratitude ARP which updates the ARP table with a new PC MAC address and
ingress port TCAM still points to an old adjacency.

Flip through those phone page parameters and you will get there.
