Security violation occurred, caused by MAC address

Hi All

Has anybody faced this Zero Mac address issue thereafter putting a port on
err-disable state on Cisco Switch 4500 the PC & Cisco IP phone connected to
Port 2/26 and PC is having Windows 7 OS

Zero Mac Address is Acting as 3rd Mac Address and the port comes to
Err-disable state.

May 3 10:05:29.082: %PM-4-ERR_DISABLE: psecure-violation error detected on
Gi2/26, putting Gi2/26 in err-disable state
May 3 10:05:29.086: %PORT_SECURITY-2-PSECURE_VIOLATION: Security violation
occurred, caused by MAC address 0000.0000.0000 on port GigabitEthernet2/26.

 Following are the configuration of Port security

interface GigabitEthernet2/26
 switchport access vlan 11
 switchport mode access
 switchport voice vlan 21
 switchport port-security maximum 2
 switchport port-security mac-address sticky
 switchport port-security mac-address sticky 0014.2119.7b84
 switchport port-security mac-address sticky 0034.11b2.2fa5 vlan voice
